Tournament Pre-Enrollment Form

This pre-enrollment form is used for either (A) a team or (B) an individual. Please fill out the top sections -- registration type, gender, team manager or player information -- and then the appropriate (A) or (B) subsection below. If you have questions about this program, please email tournament@franklinsoccerschool.com.

Note: All players must have a completed, signed, and submitted FSS Release Form in order to participate.

Zip Code * Enter the zip code of the player if this is an individual registration or of the team manager if this is a team registration (e.g. 02038).
Telephone * Enter the telephone number of the player if this is an individual registration or of the team manager if this is a team registration (e.g. 5085281111).
Team - Age Bracket If you are registering a team then please enter the age bracket in which that team would play. The 'U' is "under" as in "under 10 as of last August 1."

Team - Name If you are registering a team, please enter a team name.
Team - Number of Players If you are registering a team, please the number of players you will list (e.g. 3 or 4).
Team - Players Names If you are registering a team, please list the first and last name for each player on your team.
Individual - Date of Birth If you are registering an individual player, enter the player's date of birth so that we can properly associate the player with teammates of the same age.
